Please read these Terms and Conditions carefully before using the services of Dalton Family Dentistry or our website located at daltonfamilydentistry.com. By scheduling an appointment, visiting our office, or using our website, you agree to be bound by these terms. If you do not agree, please do not use our services.

Section 01

Our Dental Services

Dalton Family Dentistry, led by Dr. Michael Blackmon with over 23 years of experience, provides a range of dental services to patients in Dalton, GA and surrounding areas, including:

  • General dentistry (cleanings, exams, fillings, X-rays)
  • Cosmetic dentistry (whitening, veneers, smile makeovers)
  • Oral surgery (extractions and related procedures)
  • Pediatric and children’s dentistry
  • Endodontics and root canals

All services are provided by licensed dental professionals. The specific services recommended for you will be discussed during your examination and are subject to clinical judgment and your informed consent.

Appointments & Scheduling

Appointments may be scheduled by calling our office at (706) 508-4632 or by submitting a request through our website contact form. Online submissions are requests only and are not confirmed until you receive direct confirmation from our office.

Our office hours are:

  • Monday: 7:30 am – 4:00 pm
  • Tuesday: 7:30 am – 4:00 pm
  • Wednesday: Closed
  • Thursday: 7:30 am – 4:00 pm
  • Friday: 7:30 am – 4:00 pm
  • Saturday & Sunday: Closed

We ask that new patients arrive 15 minutes early to complete necessary paperwork. Please bring a valid photo ID, your insurance card (if applicable), and a list of any current medications.

Cancellation & No-Show Policy

We understand that circumstances change. If you need to cancel or reschedule your appointment, we ask that you provide at least 24 hours’ notice so that we may offer that time to another patient in need.

Repeated late cancellations or missed appointments without notice may result in a cancellation fee or dismissal from the practice. We reserve the right to require a deposit for future appointments in such cases.

Payment & Insurance

Payment is due at the time services are rendered unless prior arrangements have been made. We accept most major dental insurance plans and will file claims on your behalf as a courtesy.

  • You are responsible for any amount not covered by your insurance, including co-pays, deductibles, and non-covered services.
  • Insurance benefit estimates provided by our office are not a guarantee of payment. Final coverage is determined by your insurance carrier.
  • Balances not paid within 30 days of your statement date may be subject to a finance charge or referred to a collection agency.
  • We accept cash, check, and major credit/debit cards. Financing options may be available — please ask our front desk for details.
If you do not have insurance, we encourage you to ask about our in-office discount plans or payment options. We are committed to making dental care accessible and affordable.

Section 05

Treatment Consent

Before any dental procedure is performed, we will explain the recommended treatment, the associated risks and benefits, and any available alternatives. Your informed consent will be obtained prior to treatment.

You have the right to refuse or discontinue treatment at any time. However, we will inform you of any potential consequences of declining recommended care. Refusal of treatment does not absolve you of responsibility for fees already incurred.

By signing your intake forms or proceeding with treatment, you acknowledge that you have had the opportunity to ask questions and understand the nature of your care.

Section 06

Treatment of Minors

Patients under the age of 18 must be accompanied by a parent or legal guardian who can provide consent for treatment. The parent or guardian is financially responsible for all services rendered to the minor.

If a minor arrives without a parent or guardian for a scheduled procedure requiring consent, the appointment may need to be rescheduled. Emergency care will be provided as necessary and legally permitted.

Section 07

Dental Records

Your dental records are maintained in accordance with Georgia state law and HIPAA regulations. You have the right to request a copy of your records at any time. A reasonable administrative fee may apply for record duplication.

Records are typically retained for a minimum of 10 years for adults and until age 28 for minors (or 10 years from the last visit, whichever is later), in compliance with Georgia law.

Requests to transfer your records to another provider will be honored within a reasonable timeframe. Please contact our office with written authorization for record transfers.

Dalton Family Dentistry provides dental services in accordance with the accepted standard of care in the State of Georgia. While we strive for excellence in every procedure, dental outcomes cannot be guaranteed.

To the fullest extent permitted by law, Dalton Family Dentistry shall not be liable for any indirect, incidental, or consequential damages arising from the use of our website or from reliance on information provided therein.

If you experience a dental emergency, please call our office immediately at (706) 508-4632. If it is after hours or life-threatening, please call 911 or visit your nearest emergency room.

Governing Law

These Terms and Conditions shall be governed by and construed in accordance with the laws of the State of Georgia, without regard to its conflict of law provisions.

Any disputes arising under or related to these terms shall be subject to the exclusive jurisdiction of the courts located in Whitfield County, Georgia.

Changes to These Terms

We reserve the right to update or modify these Terms and Conditions at any time. Changes will be posted on this page with an updated effective date. Your continued use of our services after any changes constitutes your acceptance of the revised terms.
